ANNEX 1 - MANDATORY CONDITIONS:
The following conditions in Annex 1 are mandatory under the provisions of the Licensing Act 2003:
1. Alcohol shall not be sold or supplied:
(a) At any time when there is no designated premises supervisor in respect of the
premises licence, or
(b) At any time when the designated premises supervisor does not hold a personal licence or his/her personal licence is suspended
2. Every sale or supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE:
The following conditions in Annex 2 are conditions consistent with the operating schedule submitted by the applicant within the application:
(1) The licence holder shall display and maintain on the licensed premises public information notices concerning:
(a) disorder issues at the request of Suffolk Constabulary;
(b) any restrictions on admittance of individuals according to age outside the premises;
(c) conditions of entry to the premises in the vicinity of entrance to the premises;
(d) patrons leaving the area quietly.
(2) The front door of the premises shall be kept locked shut (but with a key always available and in the lock on the inside of the door for Fire escape purposes) at all times.
(3) The windows of the premises to be kept shut during the performance of any live or recorded music.
(4) The rear door of the property shall be kept shut (save for access or egress) whilst any music events (either karaoke or live bands) are taking place.
(5) The volume control of any amplification equipment being used at the licensed premises shall be under the direct control of the designated premises supervisor (DPS), or another responsible individual nominated by the DPS, on all occasions. At the direct request of an authorised officer of the Local Authority or Suffolk Constabulary the volume shall be turned down.
(6) Customers shall not be admitted, or re-admitted, to the licensed premises after 23:00hrs. (Note: This condition shall not apply to customers using bed and breakfast facilities).
(7) Glasses and opened bottles shall be retained or disposed of on the premises.
(8) No person under the age of 14 shall be in the bar of the licensed premises during the permitted hours, unless one of the following applies:
(a) He is the child of the holder of the premises licence
(b) He resides in the premises, but is not employed there
(c) He is in the bar solely for the purpose of passing to or from some part of the premises which is not a bar and to or from which there is no other convenient means of access or egress
(d) The bar is in railway refreshment rooms or other premises constructed, fitted and intended to be used bona fide for any purpose to which the holding of a premises licence is ancillary
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY:
