ANNEX 1 - MANDATORY CONDITIONS
The following conditions in Annex 1 are mandatory under the provisions of the Licensing Act 2003:
1. Alcohol shall not be sold or supplied:
(a) At any time when there is no designated premises supervisor in respect of the
premises licence, or
(b) At any time when the designated premises supervisor does not hold a personal licence or his/her personal licence is suspended
2. Every sale or supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
The following conditions in Annex 2 are conditions consistent with the operating schedule submitted by the applicant within the application:
1. Staff shall request a bona fide photographic identity proof of age where they have reason to suspect that the individual is under 18 years of age.
2. A log of all refusals shall be maintained and kept on the licensed premises.
3. The premises licence holder shall ensure that an adequate policy of public liability insurance is in force and validly maintained in respect of the licensed premises.
4. Any recorded music being played at the licensed premises shall be background music only.
5. The premises licence holder shall ensure that discounts of over 10% shall not be offered on the sale of alcoholic products.
6. The premises licence holder shall take all reasonable steps to ensure that they maintain close contact with local businesses and the Police in respect of issues relevant to promotion of the licensing objectives.
7. The premises licence holder shall ensure that at least one member of staff is first aid trained.
8. The premises licence holder shall promote the use of industry or police approved proof of age identity forms.
9. The premises licence holder shall conduct an adequate risk assessment prior to the opening of the licensed premises and then further risk assessments at least once per annum thereafter.
10. The premises licence holder shall ensure that emergency lighting and fire alarms shall be installed and maintained on the licensed premises.
11. Waste bins shall be kept secure, emptied and maintained in a clean condition.
12. Deliveries and emptying of bottles shall be kept within trading hours, wherever practicable to do so, and in any event shall not be later than 18:00hrs.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY
