ANNEX 1 - MANDATORY CONDITIONS:
The following conditions in Annex 1 are mandatory under the provisions of the Licensing Act 2003:
1. Alcohol shall not be sold or supplied:
(a) At any time when there is no designated premises supervisor in respect of the
premises licence, or
(b) At any time when the designated premises supervisor does not hold a personal licence or his/her personal licence is suspended
2. Every sale or supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE:
The following conditions in Annex 2 are conditions consistent with the operating schedule submitted by the applicant within the application:
1. No person shall be supplied with alcohol on the licensed premises unless providing bar staff with evidence of gym membership (whether permanent or temporary).
2. The premises management shall operate a 'Challenge 21' or similar photographic identity proof of age scheme, to minimise the risk of underage sales of alcohol.
3. The licence holder, or their nominated representative, shall actively participate in the local PubWatch (or similar) scheme where such a scheme operates in the locality of the licensed premises.
4. The licensed premises shall only be hired out for functions involving licensable activities to bona fide gym members, and the premises management shall ensure that the Designated Premises Supervisor, or another individual nominated by the DPS, is on site to monitor any such hirings.
5. Poledancing or similar activities shall not involve any full or partial nudity. There shall be no activities or entertainment carried on or permitted at the licensed premises that give rise to concern in respect of protecting children from harm.
6. The licence holder shall conduct regular reviews with staff to address any relevant issues arising in connection with promotion of the licensing objectives.
7. The licence holder shall liaise regularly with the local Police in order to promote the prevention of crime and disorder licensing objective.
8. The licence holder shall liaise regularly with the Fire Service and Health and Safety Officer in order to promote the public safety licensing objective.
9. The licence holder shall ensure that management and staff at the licensed premises are adequately trained to promote the protection of children from harm licensing objective.
ANNEX 3 - CONDITIONS ATTACHED AFTER A HEARING BY THE LICENSING AUTHORITY:
